Follow these steps and complete this report at the scene of the accident.
🚫 DON’T
- “Make a deal” or agree on damages privately.
- Leave the scene, even if the accident seems minor.
- Accept cash, checks, or private settlements from anyone.
- Deny injuries to yourself or your passengers.
- Offer to pay for anything, even if you believe you’re at fault.
- Administer first aid unless you are licensed to do so.
✅ DO
- Move to a safe location, such as the shoulder or another secure area, to prevent further accidents.
- Ask someone to call the police and request medical help, repeating every 5 minutes if needed.
- Remember the 3 C’s — stay Calm, Courteous, and Consistent when discussing the accident.
- Gather complete information from all parties involved.
- Fill out this report at the scene, not later.
- Get contact details of witnesses, including their names, addresses, and phone numbers.
- Notify the vehicle’s owner as soon as possible if you were driving someone else’s car.
